I hereby authorize Air Canada to make an annual payroll deduction of the current ACRA membership fee for my continuous membership in the Air Canada Recreation Association until such time that I advise the ACRA in writing that I wish to terminate my membership.

Current annual fee $15.00 . First payment to be made by e-transfer and subsequently through payroll deduction.

I hereby authorize Air Canada to deduct from my salary once a year the amount of my membership fee for the Air Canada Recreation Association until I terminate this agreement by written notice to the ARAC.

Current annual membership fee of $15.00 . The first payment is made by Interac e-Transfer and subsequently by payroll deduction.
